Lost your work permit – Can it be re-issued?

• Go to the commune/ward police office where you reside to report the loss
• Request a written confirmation of the loss from the police

• A written request from the employer for re-issuance (Form No. 11/PLI – Appendix I, Decree 152/2020/NĐ-CP)
• Two color photos (4x6cm, white background, full-face, no glasses, taken within the past 6 months)
• Police confirmation of the lost permit
• Approval document for the demand of foreign labor (if required)
• Notarized copy of a valid passport
• Other documents as required by authorities

• Submit the dossier to the Department of Home Affairs where the employee is working

What is the validity period of the re-issued permit?

According to Article 15 – Decree 152/2020/NĐ-CP: “The validity period of the re-issued work permit will be equal to the remaining validity of the lost permit.”
